F-13-501 Occurrence Reporting: Adara External Occurrence Reports
๐ฏ PURPOSE
Clinicians must complete a QI Event or Client Occurrence within 24 hrs of the occurrence. If the unusual occurrence involves a client that does not have an Electronic Health Record in HCHB, then they must follow the steps below to ensure an external Unusual Occurrence Report is completed. The supervisor may initiate the report on behalf of an employee if indicated.
๐ PROCESS
- Any client incident, accident, injury or unusual occurrence that occurs while an employee is providing care to a client or working in the clientโs home should be reported immediately to a supervisor.
- If the client has an electronic health record in HCHB, refer to F-12-500 Create QI Report in PointCare.
- If the client does not have an electronic health record in HCHB, notify the supervisor immediately.
- External Occurrence Report must be completed as directed.
- Supervisor may complete on a clinicianโs behalf if appropriate
