F-12-500 Create QI Report in PointCare

🎯 PURPOSE

This process describes how to create a QI Report in PointCare that involves non-employees, such as clients, family members, visitors, etc. Types of non-employee related occurrence types are categorized in HCHB as QI Reports and include:

  1. Client Occurrence (refer to F-12-501 QI Reporting: Client Occurrence)
    1. Vulnerable Adult/Child Reporting (refer to F-12-505 QI Reporting: Vulnerable Adult/Minor Reporting)
    2. Witnessed/Unwitnessed Falls (refer to F-12-506 QI Reporting: Witnessed/Unwitnessed Falls)
  2. Medication Errors (refer to F-12-502 QI Reporting: Medication Errors)
  3. Complaint (refer to F-12-503 QI Reporting: Complaint)
  4. Infection Occurrence (refer to F-12-504 QI Reporting: Infection Occurrence)

Any client incident, accident, injury or unusual occurrence that occurs while an employee is providing care to a client or working in the client’s home should be reported immediately to a supervisor.


πŸ”„ PROCESS

Clinician will complete a QI Report in PointCare within 24 hrs of the occurrence. The supervisor may initiate the report on behalf of an employee if indicated. The supervisor will investigate the occurrence and provide appropriate direction/follow-up. If the unusual occurrence involves a client that does not have an Electronic Health Record (EHR) in HCHB, then an external Unusual Occurrence Report document will be completed. Refer to F-13-501 Occurrence Reporting: Adara External Occurrence Reports.

NOTE: Unlicensed personnel (ULP) do not have access to PointCare QI reports. ULP will be instructed to call their supervisor to report the occurrence. Supervisor will complete QI report in HCHB.

Create Client Occurrence QI Report in PointCare:

Client Occurrence reports can be created within a visit document AND outside of the visit in the medical record in PointCare.

NOTE: If occurrence is documented inside of a visit, it will not sync to the back office until the visit is synced. If it is documented in the medical record outside of a visit, it will sync to the back office the next time the device is synced when completed.

Within a Visit:

  1. Select PRN from the Visit Action Menu:

  1. Select the menu in the upper right corner:

  1. Select Medical Records:

  1. Select the appropriate client and episode:

  1. Scroll down and select Features:

  1. Select QI Reporting:

  1. Select appropriate type:

  1. Select Add:
