O-12-825 Individual Abuse Prevention Plan / Vulnerability Pathway

🎯 Purpose

Describes how to create the Individual Abuse Prevention Plan following the Vulnerability Assessment.

The Clinician that conducts the Start of Care, Resumption of Care and/or Recertification Visit is responsible for developing the Individual Abuse Prevention Plan on the plan of care which all disciplines should review and follow. Refer to O-12-824 Vulnerability Assessment for details on how to complete the assessment.

  1. From the Visit Action Menu, select Pathways:

  2. Clinician will be prompted to select Pathways associated to β€œYes” responses in the vulnerability assessment. Select to add to the Plan of Care

    NOTE: New pathways may be customized, but it is not required

  3. Any additional pathways selected as part of the Individualized Abuse Prevention Plan in the assessment will also be highlighted. Select to add to the Plan of Care

    NOTE: If pathway is not selected, clinician will receive a prompt to review the pathway or assessment.

    Clinician should either update the pathway, or return to Physical Assessment to correct the selected Response
