O-12-824 Vulnerability Assessment

🎯 Purpose

Describes how to document the vulnerability assessment in PointCare.

🔄 Process

1. Go to Physical Assessment

   

2. Select Vulnerability

   

3. Vulnerability assessment will assess risk for vulnerability in the following areas:

   1. Self Abuse

      

   2. Physical Abuse

      

   3. Financial Exploitation

      

   4. Sexual Abuse

      

   5. Self Management- Health/Medical Needs

      

   6. Self Management- Personal Safety

      

   7. Self Management- Violent Crime/Physical Aggression

      

4. If client is not at risk for vulnerability, select No

   

5. If client is at risk for vulnerability, select Yes

   

6. Select pertinent actions to reduce vulnerability risk. Individualized measures include action taken during the visit and recommended action to incorporate into the Plan of Care

   
   

   Example:

   Clinician provided education related to indication for emergency services and instructed client/caregiver when to notify provider. Clinician recommending PT evaluation and will address fall safety precautions in the Plan of Care

   

7. Any actions selected that are associated to pathways will prompt clinician to select during Pathway development. Refer to O-12-825 Individual Abuse Prevention Plan/Vulnerability Pathway
