F-13-601 Request a Referral to a Mobile Wound Clinic

🎯Purpose:

To describe the process for requesting a referral to a Mobile Wound Clinic for consultation and assistance with a wound assessment to determine the best possible product and an acceptable visit frequency to allow maximum wound healing.


Mobile Wound Clinics offer wound consultations by a physician and/or and advance practice nurse. Adara should coordinate the home visits with the Mobile Wound Clinic so the Adara home visit and the Mobile Wound Clinic visit do NOT occur on the same day.


🔄Process:

1.      Evaluate the need for a mobile wound consult. Refer to F-13-301 Indicators for Mobile Wound Consult.

2.      Obtain verbal consent from the client to initiate a referral and release protected health information to a mobile wound clinic. Document consent was received within a Care Coordination Note

3.      Obtain an order from the home health ordering provider for a wound consult and the explain the reason as follows:

a.       The Mobile Wound Clinic provider will evaluate the wound(s) and may order new wound treatment.

b.      Ensure the primary care provider agrees with collaborating with the wound clinic provider for optimal wound healing.

c.       Inform the provider Adara will send the new orders for wound care to the primary care provider for approval to incorporate into the plan of care.

d.      Enter a Physician Order (refer to O-12-100 Physician Order Content).

4.      Contact the Clinical Supervisor/designee and request they make a referral to the Mobile Wound Clinic and the reason why. Inform them you already received the order from the primary care provider.

5.      Ensure wound care supplies are in the home for the Mobile Wound Clinic Consult. They utilize the wound supplies provided under the current home health episode.
