*Able to get to and from the toilet and transfer independently with or without a device

*Transfers SAFELY on and off the toilet without assistance

*SAFELY able to get to and from the toilet without assistance. No human assistance needed with all of the above

*If ANY assistance required to be SAFE, continue down the responses

*When reminded, assisted or supervised by another person, able to get to and from the toilet and transfer

*SAFE with assistance getting to and from as well as transferring on and off the toilet.

*If not safe with assistance, continue down the responses

*Unable to get to and from the toilet but is able to use a bedside commode (with or without assistance)

*UNSAFE with assistance getting to and from or transferring on and off the toilet but SAFE utilizing a bedside commode.

*Able to use a bedside commode SAFELY with or without assistance

*If able to SAFELY get to and from and transfer on and off the toilet with assistance it is not a 2, look at response 1

*Unable to get to and from the toilet or bedside commode but is able to use a bedpan/urinal independently

*Likely non-ambulating

*Likely bedfast

*Able to utilize a bedpan/urinal without ANY assistance

*Is totally dependent in toileting

*Unable to tolerate being out of bed

*Is dependent on another for toileting

*Is unable to get to/from the toilet, is not able to use the bedside commode or bedpan/urinal as defined in the responses, or if such equipment is not present in the home to allow assessment.


General Instructions:

  • Observe the client during mobility to and from the toilet as well as transfers and determine the amount of assistance required to be SAFE with both aspects.
  • If the client is able to perform the transfer but not safe, need to continue down the responses
  • It is not taking into account if there is a caregiver available
  • GG0170 F, Clinical Summary and therapy evaluations should correlate with this response
  • REMINDER: the item is scored PRIOR to intervention provided.
