🎯Purpose:

(M1850) Transferring: Current ability to move SAFELY from bed to chair, or ability to turn and position self in bed if patient is bedfast.

*Able to independently transfer

*Transfers SAFELY supine to sit, sit to stand, and bed to chair (independent in bed mobility and transfers)

*SAFELY ambulates to nearest sitting surface if not adjacent to the bed

*No device or human assistance needed with all of the above tasks

*Able to transfer with minimal human assistance OR with use of an assistive device

*Independent/SAFE with device, and no assistance needed, OR assistance, and no device needed

*If require both assistance AND a device, it is not a 1, continue down the responses

*"Minimal human assistance" includes verbal cueing, supervision <25% manual assist, and environmental set up

*Able to bear weight AND pivot during the transfer process but unable to transfer self

*Able to SAFELY bear weight AND pivot

*If not able to complete both, it is not a 2, continue down the responses

*Requires ANY assistance AND a device

*Requires > or = 25% manual assistance

*Able to bear weight refers to the client's ability to support the majority of his/her body weight through any combination of weight bearing-exercises

*Unable to transfer self AND is unable to bear weight OR pivot when transferred by another person

*Not bedfast (able to tolerate being out of bed)

*Likely a CHAIRFAST client

*If client is able to bear weight and pivot, it is not a 3, look at response 2

General Instructions:

  • Observe the client during transfers and determine the amount assistance required for SAFE transfers.
  • If the client is able to perform the transfer, but not safe, need to score the lower item
  • It is not transferring from the toilet or into/out of the shower
  • It is not taking into account if there is a caregiver available
  • **If there is no chair in the client's bedroom or the client does not routinely transfer from the bed directly into a chair in the bedroom, report the client's ability to move from a supine position in bed to a sitting position at the side of the bed, and then the ability to stand and then sit on whatever surface is applicable to the client's environment and need (e.g. a chair in another room, a bedside commode, the toilet, a bench, etc). Include the ability to return back into bed from the sitting surface
  • GG0170 A-E, Clinical Summary and therapy evaluations should correlate with this response
