O-12-117 Therapy Pathways for Evaluation Only
🎯 Purpose
Therapists develop an order for “Evaluation Only” type visits to address skilled interventions performed during that visit when no further visits after the evaluation will be conducted. Specific pathways are available without subsequent visit interventions for the clinician to address these visit types. Evaluation Only encounters are typically documented as an __00X (Hospice subcontract one-time visits), or an __01 (Add-On Evaluation with no subsequent visits ordered), but may also be used on a __00 document, only if there are no other visits ordered (including other disciplines).

Select Pathways:
Select PT/OT/ST Evaluation Performed. NO ADDITIONAL VISITS REQUIRED:
- Select the recommend problem statements:
- Therapy Clinical Summary: Add brief narrative to include on the order to the provider
- Vulnerability/Maltreatment Assessment: Required if assessment is __00 or __00X
Comprehensive Drug Regimen Review: Required if assessment is __00 or __00X
Select additional problem statements as they apply for any other skilled interventions provided on the evaluation visit, :
